ON TRAC (Transitioning Responsibly to Adult Care) is a province-wide initiative to support the transition of youth (ages 12-24 years) with chronic health conditions and/or disabilities, and their families, as they prepare to transfer from pediatric care and to facilitate attachment to adult health care services.
A Transition Integration & Evaluation Advisory Group started in October 2014 to help guide and support the Integration of Transition clinical guidelines, tools and practices in the ambulatory clinical settings. Membership includes: Patti Byron, Dr. Curren Warf, Tracie Northway, Diane Hart, Dr. Theresa Newlove, Louanna Atkinson, Kate Thorpe, Emily Turnbull, Michaela Watson, Elana Felgar, Sabrina Gill and members of the ON TRAC team. The steps for integration include;
1) Working with each clinical team (and their adult counterparts) to make tools condition-specific
2) Training on how to use the tools (Transition Clinical Pathway and Medical Transfer Summary)
3) Teaching staff how to help youth and families access transition checklists and online toolboxes
4) Monitoring ongoing gaps, barriers and resources required (personnel, time, other) - especially for patients with complex needs
